Job Summary

The Health Services Coordinator is a vital member of the ModernHR team, responsible for delivering high-quality health services to support children and families across our programs. This role reports to the Head of Educational Excellence (HEE) and requires a strong commitment to teamwork and effective communication.

Key Responsibilities
  • Health Services Coordination: Work with families to ensure completion of child health requirements, screenings, and assessments, and track health data to ensure compliance.
  • Medical Support: Collaborate with parents to support medical and dental screenings, and ensure follow-up treatment is completed within established guidelines.
  • Staff Support: Monitor, guide, and support staff, providers, and families with children who have medical concerns across agency programs.
  • Health Record Management: Review, evaluate, and interpret health records to assess needs and plan special programs or activities to meet these needs.
  • Confidentiality and Tracking: Maintain health records in hard copy and on ChildPlus database, ensuring an effective system of tracking, follow-up, and confidentiality.
  • Referral Follow-up: Conduct and document follow-up on all referrals to ensure needs of children are met.
  • Screening and Treatment: Work with families to ensure children's hearing and vision screenings are completed within 45 days of entry into the program.
  • Health Planning: Complete individualized health and treatment plans as needed.
  • Consultation and Training: Provide consultation and technical assistance with parents, staff, and providers regarding children's health issues, compliance, referrals, and follow-up treatment.
  • Information Sharing: Share information with staff and relevant providers to ensure coordinated services that meet the needs of individual children and families.
  • Partnerships and Recruitment: Develop and maintain partnerships with local agencies to assist families in the referral of health-related services, and assist in recruiting prospective families and enrolling children with health and nutrition concerns.
  • Reporting and Evaluation: Review, evaluate, update, and prepare reports for health services, monitor health records for completeness, accuracy, and follow-up, and report findings to the supervisor.
  • Inventory and Supplies: Maintain inventory of supplies and materials needed for health and safety purposes.
  • Program Evaluation and Planning: Participate in ongoing program evaluation, planning process, and annual self-assessment.
  • Training and Development: Plan and conduct CPR, First Aid, and other health-related training for staff, providers, and parents.
  • Safety and Compliance: Ensure safety is a priority in performing all job responsibilities and participate in PQI meetings and the Safety Committee.
Qualifications and Education
  • A minimum of a Licensed Vocational Nurse (LVN) degree, with a Registered Nurse (RN) degree preferred.
  • Two years' experience working with children in the birth to five population preferred.
  • Ability to communicate effectively – verbally and in writing.
  • Strong organizational skills.
  • Commitment to teamwork.
  • Basic computer skills required – Word, Excel, Software applications.
  • Experience working with diverse groups and populations.
  • Bilingual in English and Spanish.
  • Passionate about learning and possesses curiosity about issues affecting children and families.
  • Fingerprint background clearance (DOJ, FBI & Child Abuse Clearance Index), or approved exemption.
  • Successful completion of pre-employment physical, PPD test/risk assessment, immunizations for Covid-19 + booster, pertussis, measles, and influenza (can decline).
  • Class 'C' driver's license, proof of insurance, a safety record acceptable to ModernHR, and reliable transportation required.
