We are seeking a skilled and compassionate Program Lead to join our team at H2 Health. As a Program Lead, you will have the opportunity to make a meaningful difference in the lives of our patients while advancing your career in a supportive and dynamic environment.
About the RoleThis is a unique opportunity to work in a primarily geriatric caseload, evaluating and treating patients with a mix of general, dysphagic, and cognitive difficulties. You will have the autonomy to work 1:1 with patients, providing high-quality care and outcomes.
Responsibilities- Evaluate and treat patients with geriatric caseloads
- Develop and implement individualized treatment plans
- Collaborate with interdisciplinary teams to provide comprehensive care
- Contribute to program development and growth
- Graduate of an accredited Speech Language Pathology program
- Prior experience in outpatient therapy preferred
- Active state license to practice Speech Therapy
