We are seeking a skilled and compassionate Registered Nurse to join our team as a Nurse Coordinator in the Transitional Rehab Unit at Allied Services. As a key member of our healthcare team, you will play a vital role in coordinating patient care and providing a supportive environment for patients and their families.
Key Responsibilities- Coordinate patient care under the direction of the Director of Nursing, ensuring a high level of quality and compassion.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Provide education and support to patients and their families, promoting independence and self-advocacy.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Graduate of an accredited Nursing Program.
- Minimum of two years of experience in a long-term care setting, preferably in a leadership or coordination role.
- Current licensure as a Registered Nurse in the state of Pennsylvania.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- CPR certification and ability to maintain a safe and healthy work environment.
This role requires the ability to work in a fast-paced environment, with occasional exposure to blood and body substances. The successful candidate will be able to demonstrate the knowledge and skills necessary to provide individualized care appropriate to the age-specific needs and culture of the patients served.
