We are seeking an experienced Social Services Director to lead our Social Services Department.

The ideal candidate will possess clinical knowledge, excellent interpersonal skills, and the ability to organize and prioritize multiple tasks effectively.

Key Responsibilities:

  • Plan, organize, and direct the overall operation of the Social Services Department in accordance with federal, state, and local standards, guidelines, and regulations.
  • Welcome new residents and ensure their medically related emotional and social needs are met.
  • Interview residents and family members to obtain social history.
  • Participate in discharge planning and develop social care plans.
  • Develop written plans of care for each resident.
  • Encourage resident and family participation in care planning.
  • Perform administrative tasks, such as completing forms and reports.
  • Assure social service progress notes are informative and descriptive.
  • Refer residents and families to social service agencies when necessary.

Requirements:

  • At least 3 years of experience as a Social Worker.
  • Minimum 1 year of experience in a human services field in skilled nursing or in-patient rehabilitative facility.
  • Licensed social worker in the state of PA.
  • Skilled communicator, director, and motivator.
