The Nurse Coordinator will play a vital role in the Transitional Rehab Unit at Allied Services, providing exceptional care to patients and their families. As a key member of our healthcare team, the Nurse Coordinator will be responsible for coordinating all aspects of patient care, ensuring a seamless and efficient experience for our patients.
Key Responsibilities:
- Coordinate patient care with the Director of Nursing and other healthcare professionals to ensure comprehensive and individualized care plans.
- Provide a therapeutic environment for patients and their families, promoting optimism, encouragement, and praise for positive behavior and achievement of goals.
- Assess patient needs and develop care plans that address their physical, emotional, and social needs.
- Collaborate with the healthcare team to ensure timely and effective communication, ensuring that all patients receive the highest level of care.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
Requirements:
- Graduate of an accredited Nursing Program required.
- Minimum of two (2) years of satisfactory experience in a long-term care setting preferred.
- Possession of a current license as a Registered Nurse as issued by the Pennsylvania State Board of Nurse Examiners.
- Ability to interact and communicate effectively both verbally and in writing required.
- CPR certification must be obtained and maintained.
- Must be cleared by PA State Police criminal history check.
Working Conditions:
The Nurse Coordinator will work in a well-lit and ventilated environment, including clinic areas, nurse's stations, patient rooms, treatment areas, and dining rooms. The position involves bending, reaching, stooping, and lifting, and the ability to transport oneself throughout the facility is required. Occasional exposure to blood and body substances is possible, and exposure to needle sticks may occur.
