- Conduct thorough assessments and screenings to determine eligibility for hospice services, collaborating with healthcare professionals as needed.
- Coordinate with the interdisciplinary hospice team to develop individualized care plans that meet the unique needs of each patient.
- Communicate effectively with patients and families regarding available services, consent for treatment, and any necessary documentation.
- Maintain accurate and comprehensive electronic patient records, ensuring timely and efficient admission processes.
- Participate in hospice care conferences and meetings as needed, adhering to all clinical and ethical standards set by the organization and hospice regulations.
- Minimum of a high school diploma or equivalent; bachelor's degree in healthcare or related field preferred.
- Previous experience in healthcare, hospice, or customer service; at least 2 years preferred.
- Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
- Strong organizational and time management abilities, with proficiency in electronic medical records and Microsoft Office Suite.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
