Hospice Nurse Case Manager

Comprehensive Care for Our Geriatric Population
The Medical Team is a top-ranked healthcare services company seeking a qualified Hospice Registered Nurse Case Manager (RN) or Licensed Practical Nurse (LPN) to work with our geriatric population in their own homes. As a key member of our team, you will be responsible for comprehensive assessments, individualized care planning, patient and family education, interdisciplinary care management, and palliative care all within the home setting. You will work closely with physicians, families, and our administrative nursing team to ensure a continuum of care.
Responsibilities include:
Conducting comprehensive assessments of patients and their families
Developing and implementing individualized care plans
Providing patient and family education on disease management and self-care
Collaborating with interdisciplinary teams to ensure comprehensive care
Providing palliative care and symptom management
