Job Summary:

University Home Care is seeking a skilled professional to fill the role of Home Care Intake Coordinator in our Livonia, Michigan location. As a key member of our team, you will play a crucial role in ensuring the smooth and efficient delivery of services to our clients.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to multitask and work in a fast-paced environment. You will act as a liaison between clients, their families, and medical professionals to ensure all necessary paperwork and documentation is completed accurately and in a timely manner.

Responsibilities:
  • Receive referrals from medical professionals, clients, and their family members
  • Coordinate and schedule initial consultations with clients and their families
  • Review and assess client's needs and create a personalized care plan
  • Verify insurance coverage and billing information
  • Collaborate with medical professionals to obtain necessary documentation and medical records
  • Communicate effectively with clients and their families to address any questions or concerns
  • Maintain accurate and up-to-date client records and documentation
  • Schedule and coordinate services with the appropriate home care team members
  • Ensure compliance with all state and federal regulations
  • Assist with any additional administrative tasks as needed
Requirements:
  • High school diploma or equivalent, associate degree (Not Required)
  • Minimum of 1 year experience in a healthcare setting, preferably in home care
  • Excellent communication and interpersonal skills
  • Ability to multitask and work in a fast-paced environment
  • Strong attention to detail and organizational skills
  • Knowledge of medical terminology and insurance processes
  • Proficient in Microsoft Office and electronic medical record systems
  • Must be able to pass a background check
