Job Title: Admissions Nurse

St Croix Hospice is seeking an experienced Admissions Nurse to join our team. As an Admissions Nurse, you will play a critical role in planning, organizing, and directing hospice care for patients and their families.

Key Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine hospice needs
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and families
  • Collaborate with the interdisciplinary team to ensure seamless care coordination
  • Provide education and support to patients and families on hospice care and services
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
  • Graduate of an accredited school of nursing with a current license in the state
  • At least one year of recent experience in acute care and home care/hospice settings
  • CPR certification and ability to work in a fast-paced environment
