Job Summary
We are seeking a highly skilled Pharmacy Care Coordinator to join our team at BayCare HomeCare. As a Pharmacy Care Coordinator, you will play a critical role in ensuring the seamless delivery of pharmacy services to our patients. Your primary responsibilities will include:
- Processing new referrals and verifying all necessary information
- Entering new referrals and updates into our corporate IS systems
- Verifying referrals meet regulatory, payer, and safety requirements
- Creating pharmacy packets and communicating information to referral sources and patients
- Verifying insurance and benefits for Medicare, Managed Care, and Private Fee for Service Plans
- Utilizing correct payer and diagnoses codes
- Understanding medical criteria for securing required information from referral sources
- Entering financial information into the EHR
- Requesting and communicating authorizations to payers and documenting in EHR
- Advising patients and referral sources on Medicare and Managed Care guidelines and reimbursement issues
- Performing ICD-10 coding of referrals
- Responding to referral sources, providers, and leadership in a timely manner
- Monitoring and communicating referral source activity
Requirements
To be successful in this role, you will need:
- A high school diploma or equivalent
- 1 year of infusion-related experience, insurance verification experience, or pharmacy experience
- Computer proficiency in Microsoft Office and Excel
