About the Role

Thompson Health is seeking a skilled RN Care Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality patient care and outcomes.

Key Responsibilities
  • Collaborate with a multidisciplinary care team to identify and proactively manage the care needs of high-risk patients within the primary care practice setting.
  • Provide assessment, care coordination, advocacy, and coaching for identified patients at risk for hospital admissions or emergency room visits.
  • Utilize established criteria to identify at-risk patients and determine the drivers of risk in conjunction with the patient, family, physician, and ancillary healthcare providers.
  • Measure the impact of care coordination interventions and regularly reassess the patient's risk for incurring adverse health outcomes.
Required Skills and Qualifications
  • Ability to develop and support a practice-based care management model by providing on-site and telephonic services, including wellness, health coaching, self-management techniques, disease management, and case management.
  • Identify high-risk patients needing care management services and provide education, support, and outreach to patients over the phone and in person.
  • Interact closely with clinical teams to identify care gaps and improve clinical outcomes for patients with chronic conditions such as diabetes.
  • Active support for Thompson Health's quality programs, including PCMH and Diabetes Recognition Program.
  • Stay up-to-date with clinical practice guidelines and new developments in the management of chronic diseases, and communicate and disseminate information to staff.
  • Comply with all required Joint Commission, state, and federal regulations.
Qualifications
  • Current New York State Registered Nurse license required.
  • BLS certification.
  • Proficient in the use of Electronic Medical Records (EMR).
  • Ability to create reports and interface with EMR vendor to develop reports where none currently exist.
  • Competent in Microsoft Office products (Word, Excel, Outlook, PowerPoint).
  • Effective oral and written communication skills.
  • Patient-Centered Medical Home and Diabetes Recognition Program knowledge and experience a plus.
Experience and Education
  • Preferred Experience: 3-5 years of clinical nursing or social work experience, preferably with 3-5 years of community health experience, with the adult population.
  • Electronic Medical Record experience required.
  • Experience with database entry and reporting preferred.
  • Care Management experience preferred.
Position Pay Range

$70,000-88,000 per year.

Starting Pay: Based on experience.
