Veteran Care Management Coordinator

The Veterans Home at Holyoke is in search of a dedicated and collaborative Nursing Supervisor to take on the role of Veteran Care Coordinator. This position is integral to the nursing administration team, overseeing an assigned Care Center within our long-term care facility.

Work Schedule: This is a full-time position, requiring 40 hours per week from 7:00 am to 3:30 pm, with occasional weekend rotations. As a 24/7 facility, the Veterans Home at Holyoke may necessitate the presence of this individual during nontraditional hours when operational needs arise.

Key Responsibilities:

  • Manage daily operational requirements of the Veteran Care Center.
  • Ensure the consistent delivery of high-quality nursing care.
  • Plan for optimal staff utilization to provide nursing care around the clock.
  • Hold staff accountable for clinical practices, including documentation to meet regulatory standards.
  • Facilitate effective communication and conflict resolution to foster trust.
  • Implement innovative nursing practices to enhance Veteran care.
  • Engage in the recruitment, selection, mentoring, and evaluation of nursing personnel.
  • Maintain compliance with professional and regulatory standards in clinical operations.
  • Foster a healthy and collaborative team environment.
  • Develop and enforce nursing policies and procedures.
  • Stay informed about the needs of the veteran residents.
  • Maintain competence in geriatric nursing and serve as a clinical resource for staff.
  • Ensure staff knowledge of age-specific nursing practices for the veteran population.
  • Conduct data collection and monitoring for quality improvement.
  • Assess, plan, and evaluate nursing care and progress towards goals.
  • Utilize resources effectively while exercising fiscal responsibility.
  • Participate in clinical committees to support Veteran care.
Veteran Environment Oversight:
  • Develop a care center philosophy that reflects its uniqueness and aligns with the overall mission of the Veterans Home at Holyoke.
  • Monitor fiscal operations in collaboration with nursing leadership, ensuring accountability for personnel and budget.
  • Maintain a safe environment for residents, staff, and visitors.
  • Collaborate with interdisciplinary teams to ensure comprehensive service delivery for veteran residents.
  • Prepare reports on care center issues, trends, and progress towards objectives.
Personnel Supervision:
  • Participate in the hiring process and assist with the orientation of new staff.
  • Encourage teamwork among all staff members.
  • Monitor staff activities for adherence to standards and educational needs.
  • Facilitate regular staff meetings to share information and maintain communication.
  • Support staff development through regular performance evaluations and goal setting.
  • Guide staff in developing leadership skills through mentorship.
  • Conduct necessary disciplinary actions and document appropriately.
  • Evaluate staff performance using a structured management system.
  • Provide coverage for other Veteran Care Coordinators as needed.
  • Identify professional development goals and pursue continuing education opportunities.
Qualifications:
  • Current, active certification as a Registered Nurse in Massachusetts.
  • Valid CPR/BLS certification.
Preferred Qualifications:
  • Experience in long-term care with supervisory responsibilities.
  • Bachelor's degree or higher in nursing.
  • Certification in Geriatric Nursing or related fields.
  • Experience in a unionized environment.
  • Proven leadership skills and ability to implement sustainable change.
  • Strong communication skills and ability to manage conflict.
  • Knowledge of evidence-based practices and regulatory standards.
  • Familiarity with emergency nursing procedures.
  • Skills in evaluating nursing practices and health outcomes.
  • Proficiency in Microsoft Office applications.
