Work Schedule
Part-Time - 8-hour shifts

Monday to Friday: 7:30 AM - 8:00 PM

Rotating Weekends & Holidays: 10:00 AM - 6:00 PM

Key Responsibilities
1. Assess physician medication orders for suitability regarding drug selection, dosage, potential interactions, and administration routes. Propose therapy modifications to physicians and facility staff as necessary.

2. Maintain a comprehensive patient database, ensuring accurate current and historical medical information, relevant clinical and laboratory data, and ongoing medication records (including over-the-counter and home remedies).

3. Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, assessed, and communicated with relevant healthcare team members.

4. Collaborate with the healthcare team to provide clinical and technical assistance as required.

5. Document all interactions with physicians, nurses, other healthcare professionals, and patients/residents, including their families, in the medical records.

6. Implement policies and procedures for the safe procurement, storage, distribution, and utilization of pharmaceuticals and biologicals, including their disposal.

7. Maintain all necessary documentation in compliance with federal and state regulations and company policies.

8. Notify the Pharmacy Manager of any medication-related errors.

9. Assist facility staff with procedures related to the safe and effective management of pharmaceuticals.

10. Perform additional tasks as assigned.

Qualifications
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- Minimum one year of experience in long-term care.

Licenses/Certifications:
As mandated by governmental regulations and business requirements.

Skills:
- Detail-oriented, goal-driven, team-oriented, and effective oral communication skills.
