We are seeking a skilled Admissions Registered Nurse to join our team at Hospice of Holland Inc. As an Admissions Registered Nurse, you will play a critical role in ensuring the smooth transition of patients into our hospice care program.
Key Responsibilities- Determine patient eligibility for admission into our hospice program through thorough assessments and interviews.
- Complete the initial assessment of the admissions process to address the immediate care needs of patients and their families.
- Provide education and support to patients and their families on symptom management and end-of-life care.
- Communicate patient needs and concerns to the patient's assigned Registered Nurse Case Manager and on-call staff.
- Consult with physicians to obtain necessary orders and ensure timely delivery of care.
- Graduate of an accredited school of nursing with current Registered Nurse (RN) licensure in the State of Michigan.
- Two years of recent clinical experience preferred, but recent nursing graduates are also encouraged to apply.
- Sound clinical judgment and knowledge of pain symptom management with end-of-life care.
- Competitive salary range: $33.49 - $44.58 per hour.
- Sign-on bonus available.
