Family Care Coordinator

KVC Health Systems is seeking a highly skilled Family Care Coordinator to join our team. This role plays a vital part in ensuring the safety and well-being of children and families across Kentucky.
Key Responsibilities:- Establish initial contact with families within 24 hours of referral to assess safety and provide necessary support.
- Conduct regular team meetings and maintain open communication with family members, providers, and DCBS personnel to monitor progress and eliminate barriers to service provision.
- Facilitate the implementation of each family's Action Plan by coordinating care with all identified providers and maintaining ongoing communication to ensure timely goal attainment.
The ideal candidate will have a Bachelor's degree in Social Work or a related field, experience working with complex and multi-need families, and a valid driver's license.
