About the Role:

We are seeking an experienced Registered Nurse to join our team as a Patient Care Coordinator.

The successful candidate will be responsible for coordinating and delivering high-quality patient care, ensuring seamless transitions and excellent outcomes.

  • Responsible for the total nursing care of residents on assigned shifts
  • Assumes responsibility for compliance with Federal, State, Local, and company regulations
  • Explains procedures and treatments to residents and their families

To be successful in this role, you will need to have strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:

Delivering High-Quality Patient Care:

  • Administers prescribed medications and treatments in accordance with approved nursing techniques
  • Charts progress notes in an informative, factual manner that reflects the care administered and resident's response to care

Coordination and Communication:

  • Receives and gives report from/to nurse relieving/coming on and records sufficient information and implements appropriate follow-up action as necessary
  • Maintains awareness of comfort and safety needs of residents

Collaboration and Teamwork:

  • Participates in department or unit quality improvement activities
  • Follows established procedures for charting and reporting all reports of incident/accidents for residents and employees

Educational Requirements:

A current State of LVN/LPN license with a minimum of 2 years related experience; or equivalent combination of education and experience. Good standing in the state registry is required.
