Job Summary

Zepf Center is seeking a full-time Health Navigator Case Manager to provide direct services to help achieve better health and wellness. The ideal candidate will be an advocate for consumers to obtain healthcare and other treatment services.

Key Responsibilities

  • Provide direct services for consumers to help them achieve healthcare goals and improve health and wellness.
  • Maintain clinical responsibility for assigned clients with mental health diagnoses or co-occurring mental health and substance use disorders.
  • Develop and update individualized action plans for each client based on case management assessments.
  • Provide strength-based assessment of patients for care coordination needs, abilities, preferences, and goals.
  • Facilitate warm hand offs and provide therapeutic client-centered evidence-based interventions.
  • Monitor clients to identify potential relapse, changes in treatment needs, and effectiveness of the individualized action plan.
  • Complete required paperwork in accordance with timelines and standards required by the agency, regulatory bodies, and payors.
  • Assist patients in gaining access to essential community resources via transportation using Zepf fleet vehicles.
  • Advocate on behalf of patients to obtain appropriate treatment and services.
  • Work with healthcare, wellness, and mental health professional staff to achieve treatment goals.

Requirements

  • Bachelor's degree in social work, counseling, public administration, nursing, or related field.
  • LPN or LSW preferred.
  • High School Diploma, GED, or Associates degree plus at least three years' experience with care coordination will also be considered.
  • Military veteran status is required for this position.
