Zepf Center is a leading provider of behavioral health and substance use disorder services in Northwest Ohio. We are seeking a Full-Time Case Manager (Health Navigator) to provide direct services to help individuals achieve better health and wellness. The successful candidate will advocate for consumers to obtain healthcare and other treatment services, ensuring their needs are addressed.

The Case Manager (Health Navigator) will provide a wide range of community support and coordination of services to individuals experiencing serious mental illness. They will assist clients with the development of short and long-term goals and the acquisition of community resources. Services will be provided with respect and in accordance with the client's rights.

Key Responsibilities:

  • Provide direct services for consumers to help them achieve healthcare goals and improve health and wellness.
  • Maintain clinical responsibility for assigned clients with mental health diagnoses or co-occurring mental health and substance use disorders.
  • Develop and update Individualized Action Plans (IAPs) for each client based on case management assessments and reviews.
  • Provide strength-based assessments, identify objectives and interventions, and facilitate IAP updates, referrals, and linkage to appropriate resources.
  • Facilitate care coordination, treatment planning, discharge planning, and service coordination.
  • Monitor clients to identify potential relapse, changes in treatment needs, and barriers to achieving goals.
  • Complete required paperwork and maintain accurate records.
  • Advocate on behalf of patients to obtain appropriate treatment and services.
  • Collaborate with healthcare, wellness, and mental health professional staff to achieve treatment goals.

Requirements:

  • Bachelor's degree in social work, counseling, public administration, nursing, or related field, LPN or LSW preferred.
  • High School Diploma, GED, or Associate's degree plus at least three years of experience with care coordination will also be considered.
