At Zepf Center, we are seeking a dedicated Full-Time Case Manager (Health Navigator) to join our Grant Team. This role is responsible for providing direct services to help individuals achieve better health and wellness.

The Case Manager (Health Navigator) will advocate for consumers to obtain healthcare and other treatment services, ensuring their needs are addressed. They will assist clients with the development of short and long-term goals and the acquisition of community resources.

Key Responsibilities:

  • Provide direct services for consumers to help them achieve healthcare goals and improve health and wellness.
  • Maintain clinical responsibility for assigned clients with mental health diagnoses or co-occurring mental health and substance use disorders.
  • Develop and update Individualized Action Plans (IAPs) for each client based on case management assessments and reviews.
  • Provide strength-based assessments, identify objectives and interventions, and facilitate IAP updates, referrals, and linkage to appropriate resources.
  • Facilitate treatment planning, discharge planning, service coordination, individual/group/family counseling, transition planning, wellness programming, resource access, advocacy, recovery planning, healthcare management, and care coordination/team meetings/training.
  • Monitor clients to identify potential relapse, changes in treatment needs/goals, and barriers to achieving goals.
  • Complete required paperwork in accordance with agency, regulatory, and payor standards.
  • Provide assistance to patients in gaining access to essential community resources via transportation and documentation.
  • Advocate on behalf of patients to obtain appropriate treatment and services.
  • Collaborate with healthcare, wellness, and mental health professional staff to achieve treatment goals.
  • Keep records on care management and collect data to maintain outcome measures.
  • Attend required training to ensure individual and professional growth.

Requirements:

  • Bachelor's degree in social work, counseling, public administration, nursing, or related field, LPN or LSW preferred.
  • High School Diploma, GED, or Associate's degree plus at least three years of experience with care coordination will also be considered.
