Job Title: Patient Health Navigator

Zepf Center is a leading provider of behavioral health and substance use disorder services in Northwest Ohio. We are seeking a skilled Patient Health Navigator to join our team.

Job Summary:

The Patient Health Navigator will provide direct services to help patients achieve better health and wellness. This includes advocating for patients to obtain healthcare and other treatment services, coordinating care, and providing support to patients and their families.

Key Responsibilities:
  • Provide direct services to patients to help them achieve healthcare goals and improve health and wellness.
  • Maintain clinical responsibility for assigned patients with mental health diagnoses or co-occurring mental health and substance use disorders.
  • Develop and update individualized action plans for patients based on assessments and identified needs.
  • Provide strength-based assessments and interventions, including treatment planning, discharge planning, and resource access.
  • Monitor patients to identify potential relapse, changes in treatment needs, and effectiveness of action plans.
  • Complete required paperwork and documentation in accordance with agency and regulatory standards.
  • Assist patients in gaining access to essential community resources and provide transportation using Zepf fleet vehicles.
  • Advocate on behalf of patients to obtain appropriate treatment and services.
  • Collaborate with healthcare, wellness, and mental health professional staff to achieve treatment goals.
Requirements:
  • Bachelor's degree in social work, counseling, public administration, nursing, or related field, LPN or LSW preferred.
  • High School Diploma, GED, or Associate's degree plus at least three years of experience with care coordination will also be considered.
