Position Overview:
The Department of Veterans Affairs is seeking a dedicated Pharmaceutical Care Specialist to serve as a mid-level practitioner. This role involves the design, implementation, and monitoring of therapeutic drug plans aimed at achieving specific health outcomes through direct patient and provider interactions.

Key Responsibilities:
1. Conduct physical assessments and interpret necessary measurements to support and adjust patient medication therapies.
2. Order and review laboratory tests and diagnostic studies to ensure optimal medication management.
3. Maintain timely follow-ups with patients to monitor drug therapy responses, adverse reactions, and adherence to treatment plans.
4. Utilize motivational interviewing techniques to gather patient-specific data and conduct thorough medication history assessments.

Patient Education:
Effectively instruct patients and their families on the proper use of medications and medical devices, adjusting communication methods based on individual needs and comprehension levels.

Emergency Assistance:
Assist in medical emergencies and high-risk situations as required.

Medication Management:
Ensure appropriate selection of medications, dosages, and administration routes. Provide recommendations for alternative medications when necessary.

Documentation and Compliance:
Utilize the VAADERs Adverse Drug Reaction reporting system to monitor and report medication errors and compliance issues. Document all pharmaceutical care plans and interventions in accordance with facility policies.

Collaboration:
Work closely with healthcare teams to support effective drug usage criteria and ensure quality patient care in a cost-effective manner.

Order Processing:
Process electronic orders accurately, verifying patient information and medication details while checking for allergies and potential drug interactions.

Preceptorship:
Serve as a mentor for pharmacy students and residents, maintaining qualifications in line with professional standards.

Work Schedule:
This position operates on a standard schedule with opportunities for ad-hoc telework.

Qualifications:
Applicants must be U.S. citizens and possess a full, unrestricted license to practice pharmacy. A Doctor of Pharmacy degree from an accredited institution is required. Candidates should have relevant experience in clinical pharmacy practice, particularly in women's health and PACT.

Physical Requirements:
Must meet the physical demands outlined in VA Directive and Handbook.
