Job Summary

The Clinical Pharmacist-Outpatient is a key member of the pharmacy team, responsible for providing pharmaceutical care and pharmacotherapy services to patients in an outpatient setting. This role requires a strong understanding of medication therapy, patient assessment, and communication skills to work effectively with healthcare teams.

Responsibilities
  • Provide pharmaceutical care and pharmacotherapy services to patients in an outpatient setting
  • Assess patient medication regimens and make recommendations for improvement
  • Communicate with healthcare teams to ensure safe and effective medication use
  • Participate in multidisciplinary team rounds and provide drug information and recommendations
  • Document patient care plans and medication therapy in the medical record
  • Assume limited prescribing patient care management functions under appropriate supervision
  • Provide consultative services to patients and healthcare staff regarding medication therapy
  • Design and participate in quality assurance projects and research studies involving drug therapy or clinical pharmacy functions
Requirements
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
  • Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
  • Ability to communicate orally and in writing to both patients and healthcare staff
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security
Work Schedule

8:00AM-4:30PM with rotating tours, weekends, and holidays as needed for patient care
