Patient Care Liaison

Position Summary
The Patient Care Liaison serves as a vital link between patients, healthcare providers, and the broader medical system. This role is essential in ensuring that patient requirements, discharge strategies, and care coordination align with established care management standards. The Patient Care Liaison must be attentive to patient needs, care levels, medical conditions, discharge protocols, and medication management. Responsibilities may also encompass patient education, care guidance, and support.
Key Responsibilities
Care Coordination:
- Facilitates patient navigation through the healthcare landscape, acting as an advocate and guide.
- Ensures seamless continuity of care with external healthcare entities and facilities.
- Maintains continuity of care for patients and their families/caregivers post-hospitalization, discharge, and Emergency Department visits.
- Identifies and communicates care obstacles to the supervising physician or designated personnel.
- Conducts thorough preventive assessments for patients and assists support staff in daily patient interactions as necessary.
- Engages in the implementation of Enhanced Care Management (ECM) to guarantee that eligible beneficiaries receive comprehensive care management and coordination.
- Encourages patient self-management of health conditions and promotes behavioral changes for self-improvement.
- Fosters effective communication among interdisciplinary team members by ensuring clarity regarding patient care plans.
- Oversees patient medication management in accordance with established protocols.
- Contributes as a collaborative team member in data collection, health outcomes assessment, clinical audits, and practical evaluations.
- Participates in the review of clinical care, resource utilization, and the development of new clinical tools, forms, and procedures.
- Under the guidance of the Population Health Manager and Health Programs Supervisor, prioritizes projects for the organization.
- Coordinates project timelines and plans.
- Provides direction and support to project teams as needed.
- Monitors project deliverables using appropriate management tools.
- Identifies potential project risks and suggests suitable solutions.
- Defines project metrics, physician dashboards, and quality indicators.
- Assists in the preparation and submission of invoices for the Medical Group.
- Performs additional duties as assigned.
Qualifications
Education:
- High School Diploma required.
- Bachelor's degree preferred.
Experience:
- Minimum of 1 year of customer service experience in a healthcare environment or similar.
- A collaborative team player capable of adhering to systems and protocols to achieve shared objectives.
- Strong organizational skills and well-developed verbal and written communication abilities.
- Confidence in outreach and communication with other community healthcare organizations and personnel.
- Exhibits sound judgment, decision-making, and problem-solving capabilities.
Special Skills:
- Proficient in Microsoft Office, particularly Word and Excel.
- Strong critical thinking and resourcefulness.
- Demonstrated leadership qualities.
- Understanding of healthcare delivery systems.
- Bi-lingual in Spanish is advantageous but not mandatory.
