Clinical Nurse Leader

As a pivotal member of the healthcare team, the Registered Nurse is responsible for delivering high-quality nursing care, employing the nursing process in alignment with established standards, policies, and procedures. This role requires adaptability to evolving healthcare systems and a commitment to excellence in clinical practice, while treating all colleagues, patients, families, and healthcare team members with respect and dignity.Key Responsibilities:

- Nursing Process:
- Develops and/or contributes to individualized care plans that encompass assessment, planning, implementation, and evaluation.
- Ensures care plans reflect multidisciplinary collaboration, consultation, and education.
- Confirms that care plans are informed by admission data, ongoing assessments, age-appropriate care, cultural considerations, and appropriate acuity.
- Discusses care plans with patients and their families, ensuring timely completion.
- Monitors patient progress according to the established plan.
- Adjusts care plans as needed based on patient condition and evaluation of progress.
- Ensures care aligns with established standards.
- Facilitates the development and execution of discharge plans, addressing barriers and providing patient/family education.
- Ensures patients understand discharge instructions, identifying barriers to outpatient management.
- Prioritizes patient safety concerning medications and procedures, utilizing best practices to prevent falls, pressure ulcers, and infections.
- Ensures effective pain management and patient comfort; addresses patient anxieties and concerns regarding their condition and treatment.
- Communicates medication purposes, side effects, and administration instructions clearly to patients during their stay and at discharge.
- Patient Care Experience:
- Implements customer service standards as defined by the organization.
- Responds promptly to patient requests and alarms.
- Facilitates referrals and assists patients in accessing resources.
- Maintains patient confidentiality.
