Job Summary:We are seeking a skilled Clinical Chaplain to provide spiritual support and counseling to home care patients, families, and staff in a culturally inclusive manner. As a member of the interdisciplinary patient care team, you will participate in patient care planning and document all pertinent interventions in the patient care record using established charting methodologies.
Key Responsibilities:- Administrative Duties:
- Participate in Quality Assurance Activities, including committees for Patient Satisfaction Scores, QI Projects, and UBT.
- Collaborate with agency leadership on various committees, such as Bioethics, EAP, LCP, and ELOA.
- Conduct in-services and CEU programs to improve the Integrated Model of Care, including New Employee Orientation and Interdisciplinary Meetings.
- Direct Patient Care:
- Provide spiritual support and counseling to home care patients, families, and staff in a culturally competent manner.
- Participate in creating patient care plans that include acuity mapping to define frequency of visits/duration, document interventions/outcomes, and meet TJC/regulatory guidelines of Hospice/Homecare.
- Support the admission/consent process in collaboration with the inter-disciplinary team in explaining hospice benefits to patients and families.
- Support bereavement care plans for surviving family members for up to 13 months after patient death, including phone support, in-home visitations, bereavement support groups, and memorial services.
- Experience:
- Minimum of one (1) year of experience as a Clinical Chaplain/Spiritual Counselor in a Hospital, Ambulatory, Home Health, Palliative Care, or Hospice setting.
- Education:
- Masters Degree in Divinity, Theology, or related field from an accredited Seminary, College, or University.
- License, Certification, Registration:
- Driver's License (California)
- Basic Life Support required at hire
- Two (2) or more years of experience as a Clinical Chaplain/Spiritual Counselor in a Hospital, Home Health, Palliative, or Hospice Setting.
- Board Certified Chaplain Certification from Association of Professional Chaplains.
- Minimum of four (4) units of Clinical Pastoral Education from an ACPE accredited program.
- PhD, DMin, ThD, or EdD from an accredited Seminary, College, or University, with certificates specializing in Palliative or Hospice Care.
