Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at HRA. As a key member of our healthcare team, you will provide high-quality care to our residents, ensuring their physical and emotional well-being.

Key Responsibilities
  • Provide direct and individualized nursing care to assigned residents, adhering to nursing standards and under the supervision of the Director of Resident Care.
  • Maintain compliance with all state and federal regulations, as well as the standards of HRA.
  • Ensure the quality and safety of health services delivered to residents.
  • Implement plans of care formulated by physicians and the Director of Resident Care.
  • Document care provided, diagnoses, and plans of care accurately and in a timely manner.
  • Administer medications as ordered by physicians and document properly on the Medication Administration Record (MAR).
  • Observe for any adverse reactions and notify physicians accordingly.
  • Assemble resident charts and records, ensuring all necessary forms are complete and accurate.
  • Prepare incident/accident reports for residents as needed, notify physicians and family members, and document appropriately.
  • Maintain a safe and healthy working environment, using safe work methods and procedures in accordance with clinical standards.
  • Assist residents in learning appropriate self-care activities.
  • Organize and monitor associates and material resources necessary for meeting the needs of residents under their care.
  • Recognize emergencies and life-threatening situations, and initiate appropriate action.
  • Maintain confidentiality and resident rights regarding all resident personal information.
  • Perform any other duties requested by supervisors.
Qualifications
  • Graduated from an accredited program for LPN/LVN and currently registered in the state of the community.
  • Must obtain valid Continuing Education credits as required by the state of the community.
  • Training in Alzheimer and Dementia with a focus on Validation Therapy.
  • Minimum of one (1) year nursing experience in a healthcare facility.
  • Experience in a long-term care community and/or geriatric nursing facility preferred.
  • Experience in emergency care and first-aid.
  • Able to communicate effectively with all levels of management, associates, residents, family members, and outside contacts.
  • Experience with Microsoft Office and Outlook software preferred.
  • Able to make independent decisions.
  • Able to seek out new methods and willing to incorporate them into existing practices when applicable.
  • Must possess a passion to work with and around senior citizens.
