Job Summary

Tri-County Human Services is seeking a skilled professional to join our team as a Case Manager. This role is responsible for providing comprehensive case management services to clients, ensuring their needs are met and they receive the necessary support to achieve their goals.

Key Responsibilities
  • Client Intake and Assessment
    • Conduct thorough interviews with clients to gather information and assess their needs.
    • Develop and implement individualized treatment plans to address client goals and objectives.
  • Case Management and Coordination
    • Collaborate with treatment teams to ensure clients receive comprehensive services.
    • Coordinate with community resources to provide clients with access to necessary support services.
  • Documentation and Reporting
    • Maintain accurate and up-to-date records of client interactions and progress.
    • Prepare and submit reports as required by management and regulatory agencies.
  • Education and Training
    • Develop and implement educational programs to promote client knowledge and skills.
    • Provide training and support to clients to enhance their ability to manage their treatment plans.
  • Community Liaison
    • Establish and maintain relationships with community partners to ensure access to resources and services.
    • Represent Tri-County Human Services in community settings to promote the organization's mission and services.
Requirements
  • Education and Experience
    • High school diploma or equivalent required; one year of experience in a mental health or addictions treatment setting preferred.
  • Knowledge and Skills
    • Knowledge of addictions treatment and mental health treatment programs and services.
    • Ability to work effectively with diverse populations and provide culturally sensitive services.
    • Strong communication and interpersonal skills.
