Registered Nurse Care Coordinator (Hybrid)

Key Responsibilities:

  • Manage Transitions of Care for a designated group of patients, including follow-ups from emergency and hospital visits.
  • Offer triage support and expert consultation to team members, addressing complex cases.
  • Create comprehensive care plans utilizing the 5Ms Geriatric best practice framework.
  • Assess patient needs through the lens of Social Determinants of Health.
  • Identify and address barriers to patient engagement with essential resources.
  • Educate patients on managing chronic health conditions and available behavioral health options.
  • Act as a bridge between patients and healthcare providers, facilitating navigation of healthcare systems.
  • Initiate care planning and coordinate action steps for high-risk patients with the interdisciplinary team.
  • Empower patients to achieve their health goals through self-determination.
  • Refer patients to appropriate services and resources.
  • Lead interdisciplinary team meetings as necessary.
  • Evaluate family dynamics and conduct family meetings when appropriate.
  • Contribute to the development and delivery of team training materials.
  • Facilitate group psychoeducation and support sessions.
  • Perform additional duties as required.
  • Participate in interdisciplinary reviews and coordination for complex cases.
  • Ensure patient confidentiality in compliance with HIPAA regulations.
  • Document patient interactions in the medical record system promptly.
  • Adhere to policies regarding safety, infection control, and attendance.

Required Qualifications:

  • Active Registered Nurse (RN) license.
  • At least 4 years of experience in human services and community resource navigation.
Preferred Qualifications:

  • Knowledge of state Medicaid guidelines and application processes.
  • Experience with patients facing behavioral health challenges and substance use disorders.
  • Background in conducting home visits and understanding field safety protocols.

Working Conditions:
