Position Summary

The Case Manager at Created Women, Inc. is dedicated to providing essential support services to individuals who have survived human trafficking. This role involves conducting initial assessments to establish personalized goals and developmental plans for each client. The Case Manager will facilitate access to vital resources, employment opportunities, and long-term housing solutions, guiding clients through the various phases of our program.

Key Responsibilities
  • Deliver trauma-informed care to clients who have faced human trafficking.
  • Conduct intake activities and manage all related services.
  • Maintain a comprehensive understanding of community resources and effectively network with service providers.
  • Assist clients in connecting with resources for job placement, legal aid, housing, and medical treatment.
  • Provide counseling on issues such as mental health, unemployment, and social adjustment.
  • Gather necessary information to support client needs.
  • Conduct follow-ups on client placements as mandated by the agency.
  • Collaborate with internal and external stakeholders to enhance service delivery.
  • Meet individual productivity targets while contributing to team objectives.
  • Maintain accurate and up-to-date case records in compliance with agency guidelines.
  • Prepare required reports in a timely manner.
  • Perform additional duties as assigned by the Supervisor.

Education and Experience

  2. Experience in anti-trafficking services is a plus.
  3. Willingness to travel as required for the role.
