Position Summary

The Case Manager at Created Women, Inc. is responsible for delivering essential services to assist survivors of human trafficking in their reintegration into society. This role involves conducting initial assessments to establish personalized goals and developmental plans for each client. The Case Manager will enhance the social functioning of clients and support their overall well-being by facilitating access to necessary services, employment opportunities, and long-term housing solutions throughout the program's four phases.

Key Responsibilities
  • Provide trauma-informed care to women who have experienced human trafficking.
  • Conduct intake activities and manage all related services.
  • Possess a comprehensive understanding of community resources and establish connections with service providers for client referrals.
  • Guide clients to community resources for job placement, legal aid, housing, medical treatment, and financial assistance, offering clear instructions on how to access these services.
  • Offer counseling on various issues, including mental health, unemployment, and social adjustment.
  • Gather additional information to support client needs.
  • Perform follow-ups on client placements as required.
  • Collaborate effectively with internal and external stakeholders.
  • Meet individual productivity goals while contributing to team objectives.
  • Maintain accurate and up-to-date case records in compliance with agency guidelines.
  • Prepare reports as needed in a timely manner.
  • Undertake additional duties as assigned by the Supervisor.

Education and Experience

  2. Experience in anti-trafficking initiatives is a plus.
  3. Must have reliable transportation and valid vehicle insurance.
