Key Responsibilities
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Provide direct and indirect care to patients, including medication management, wound care, and education on disease management.
  • Collaborate with healthcare teams to ensure seamless care transitions and coordination.
  • Conduct thorough assessments of patients' physical, emotional, and social needs.
  • Develop and implement plans to address patients' needs, including referrals to community resources and services.
  • Monitor patients' progress and adjust care plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure understanding and cooperation.
Requirements
  • Current RN licensure in the State of Ohio.
  • 1-2 years of experience in home health care or a related field.
  • Strong assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and CPR certification.
Preferred Qualifications
  • Bachelor's degree in Nursing.
  • Experience working with diverse patient populations.
  • Bi-lingual skills.
