Urban Pathways is seeking a highly skilled and compassionate Senior Case Manager to join our team. As a key member of our social services department, you will provide clinical and case management services to clients, coordinating medical and mental health assessments, medication management, and housing applications.
Responsibilities:- Provide information and guidance to case managers, client services, and community resources.
- Assign caseload to case managers.
- Support and guide case managers in executing all housing-related tasks, including 2010E applications, General Population applications, housing searches, and housing interviews.
- Offer comprehensive case management services for assigned clients, including entitlements, housing, employment/education, vocational training, health, mental health treatment, and substance abuse services.
- Provide counseling and support to clients on an ongoing basis, including crisis intervention, training, and support on daily living skills, assistance in problem-solving, conflict resolution, and advocacy.
- Conduct client intakes, assess client needs, develop service plans, and/or service referrals to achieve self-sufficiency.
- Conduct field work, including home visits, outreach, and escorting clients to appointments.
- Monitor client medications and coordinate all required medical and mental health appointments for clients.
- Generate program reports and oversee accuracy of client information in client database systems.
- Liaise with funding agencies, community organizations, and housing providers regarding all housing-related services.
- Facilitate client groups and staff training as required.
- Supervise incident reporting and other client activities to ensure proper documentation and data entry.
- Maintain and submit all necessary documents and reports as required by Urban Pathways, regulatory, and funding agency policies, procedures, and regulations.
- Attend staff meetings, trainings, and supervisory sessions as required.
- Develop and maintain relationships with community services and resources.
- Perform all other duties as assigned.
- Bachelor's degree required; Bachelor's degree in Social Work or related field preferred.
- Minimum two years of clinical, individual, or group counseling experience preferred.
- Experience with homeless, person living with mental health condition, and substance abuse population preferred.
- Knowledge of NYC housing subsidy programs.
- Strong computer skills and knowledge of Microsoft Office (e.g., Word, Outlook, Excel, etc.).
- Strong written and verbal communication skills.
- Strong team building and coaching skills.
- Strong problem-solving skills and resourcefulness.
- Strong interpersonal skills and ability to work independently and in a team.
- Foreign language a plus.
