Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team at AccentCare, Inc. in the Home Health setting.

Key Responsibilities
  • Provide high-quality patient care and coordinate services to meet the unique needs of each patient.
  • Conduct thorough assessments and develop individualized care plans to ensure optimal patient outcomes.
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless care transitions and coordination.
  • Monitor patient progress, identify potential issues, and implement interventions to prevent complications.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and care coordination.
Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN in a home health or similar setting.
