The Registered Nurse Case Manager PRN is a vital role in our healthcare team, responsible for coordinating patient care across the continuum. This position requires a strong understanding of medical necessity, length of stay, and timely utilization of services.

Key Responsibilities
  • Collaborate with healthcare professionals to ensure quality care and cost-effective outcomes.
  • Review patient activities to identify medical necessity and appropriateness of hospitalization.
  • Develop and implement care plans to promote timely throughput, safe discharge, and prevent avoidable readmissions.
  • Provide education and support to patients and their families to ensure a smooth transition to home or next level of care.
Requirements
  • Associate Degree in Nursing, required
  • Bachelor's Degree in Nursing, preferred
  • Case Management certification, preferred
  • Current Texas licensure with State Board of Nurse Examiners as an RN
  • 1+ years experience in healthcare
