Responsibilities:
- Supervise and manage patient care episodes from admission to discharge, ensuring seamless transitions and optimal patient outcomes.
- Coordinate patient care with other clinical staff and medical providers to ensure comprehensive and coordinated care.
- Perform home health admission visits, resumptions, recertifications, and discharges, assessing patient needs and developing individualized care plans.
- Record, assess, and observe patient progress, reactions, and symptoms, documenting all findings in accordance with agency, state, and federal guidelines.
- Assess patient needs and develop plans of care based on comprehensive assessments, ensuring patient-centered care and optimal outcomes.
- Check vital signs, administer medications, and provide wound care as needed, ensuring patient safety and comfort.
- Provide patient education and support, empowering patients to manage their health and make informed decisions.
- Document care in patient records, maintaining accurate and up-to-date records of patient interactions and care provided.
Requirements:
- Graduation from an accredited nursing school and a current, valid RN license registered in the state of Texas.
- OASIS training and certification.
- Valid driver's license and willingness to travel throughout assigned territory.
- At least one year of home health nursing experience, with a proven track record of providing high-quality patient care and achieving positive outcomes.
