  • Coordinate Patient Care: Collaborate with other clinical staff and medical providers to develop and implement comprehensive care plans that meet the patient's unique needs.
  • Home Health Admission Visits: Conduct thorough assessments and develop personalized care plans for patients, including resumptions, recertifications, and discharges.
  • Record and Assess Patient Progress: Accurately document patient progress, reactions, and symptoms, and assess the effectiveness of care plans.
  • Develop and Implement Care Plans: Create and implement care plans based on patient assessments, ensuring that patients receive the highest quality care.
  • Provide Education and Support: Educate patients and their families on disease management, medication adherence, and healthy lifestyle habits, as recommended by physicians.
  • Document Care: Maintain accurate and detailed records of patient care, adhering to agency, state, and federal guidelines.
Requirements:
  • Education and Licensure: Graduation from an accredited nursing school and a current, valid RN license registered in the state of Texas.
  • OASIS Training: Completion of OASIS training program.
  • Driver's License: Possession of a valid driver's license.
  • Travel Requirements: Willingness to travel throughout assigned territory.
  • Experience: At least 1 year of home health nursing experience required.
