Case Manager and Housing Advocate

- Provide housing support services to individuals with mental illness, including coaching, advocacy, information, and referral;
- Collaborate with community partners to identify housing opportunities and facilitate client placement;
- Develop and implement individualized plans to promote independent living skills and ensure long-term housing stability;
- Work closely with case managers and other healthcare professionals to ensure seamless service delivery and effective care coordination;
- Participate in regular team meetings and contribute to the development of best practices and policies related to housing support services.
