Case Manager

Columbia River Mental Health Services is seeking an experienced Case Manager to join our team in providing comprehensive behavioral health and recovery services. This role requires strong communication and interpersonal skills to effectively collaborate with clients, community partners, and internal teams. As a housing case manager, you will be responsible for managing referrals for housing options, developing treatment plans, and providing ongoing support to clients in maintaining stable housing.
Salary Information
The estimated salary for this position is $24.00-$28.00 per hour, based on experience and qualifications. In addition to a competitive salary, we offer a range of benefits, including health insurance, paid time off, and opportunities for career development and education.
Responsibilities
- Develop and implement treatment plans that address various life domains, including mental health, alcohol/drug issues, activities of daily living, work/school, family/natural support, income/finances, social/friends/fun, legal: medical and dental, housing/shelter, cultural/spiritual, and other.
- Manage referrals for housing options through the VHA and Key properties, including Central Park Place (SRO), Rhododendron Place, Teeny Creek.
- Work collaboratively with clients to achieve their recovery goals and provide ongoing support to ensure they remain stably housed.
