Sea Mar Community Health Centers is a community-based organization committed to providing quality, comprehensive health, human, housing, educational and cultural services. We are seeking a Full-time Case Manager for our Housing Program in Vancouver, WA.
Job Summary:
This position is responsible for their own caseload, providing mental health team consultations on housing resources, designing and implementing housing support interventions, and referring clients to established community providers.
About You:
We are looking for a highly motivated individual with excellent communication and interpersonal skills. The ideal candidate will have experience in case management, preferably in a mental health setting, and a strong understanding of housing resources and support services.
Responsibilities:
- Participate in outreach services to prospective clients, coordinate efforts with outside agencies, and work collaboratively with co-workers and other agencies.
- Provide peer counseling and support, drawing on common experiences as a peer, to validate client's experiences and provide guidance and encouragement to take responsibility and actively participate in recovery.
- Serve as a mentor to promote hope and empowerment.
- Conduct ongoing psychosocial assessments to identify psychological, social, vocational, systemic, and economic goals and needs of consumers.
- Monitor, compile, and submit data, reporting, and paperwork for the PATH program, and assist clients in obtaining housing.
