Healthcare Advocate Manager

We are growing and seeking a skilled professional to fill the role of Healthcare Advocate Manager at Columbia River Mental Health Services.
This position offers a competitive salary range of $23.92-$26.57 per hour and an array of benefits designed to support your well-being and career growth.
About the Job:
- You will serve as the liaison between our organization and in-patient facilities to arrange Face-to-Face contacts with consumers within 2 business days.
- Following the meeting, you will maintain contact with the consumer and in-patient facility to facilitate discharge planning and assist clients with overcoming potential barriers to attending outpatient appointments.
Responsibilities:
- Coordinate and mobilize treating and/or authorizing entities to ensure seamless care delivery.
- Develop and maintain relationships with community partners to promote marketing activities and coordination of care.
Qualifications:
- Bachelor's degree in social services or related field.
- Approximately 1-2 years of experience in the social service field with mental illness or chemical dependency preferred.
- Excellent communication and interpersonal skills, ability to build therapeutic relationships, and maintain confidentiality.
