Key Responsibilities:
  • Coordinate services to ensure clients make meaningful progress toward increasing earnings and securing stable housing.
  • Develop and amend Housing Stabilization Plans and DIRPs, helping clients overcome barriers to housing stability and self-sufficiency.
  • Build trusting relationships with clients, monitoring their progress on the housing stability journey.
  • Conduct outreach to referred clients, engage them in 2Gen activities, and re-engage those who have stopped participating.
  • Identify goals and activities for housing stability, barrier removal, and career-pathways, and coordinate with TEP providers to offer education and occupational training programs.
  • Maintain case notes on all client contacts.
Requirements:
  • Bachelor's degree in social work, psychology, sociology, counseling, or a related field, with one year of experience providing case management services.
  • Ability to work with clients in a caring and respectful manner, with understanding of and consideration for their unique circumstances.
  • Effective communication and interpersonal skills, with the ability to build rapport and trusting relationships.
  • Computer literate and proficient at using Microsoft Office.
