About the Role:

Theoria Medical is seeking an Advanced Practice Clinician to provide high-quality, patient-centered care to patients residing in skilled nursing facilities.

Key Responsibilities:

  • Rounding and managing the clinical care of patients at assigned SNFs
  • Prescribing medications and treatments as needed
  • Providing comprehensive primary care services
  • Timely documentation of patient care/encounters
  • Available for phone/secure messaging consultations with facility nursing staff

Requirements:

  • Nurse Practitioner licensure
  • Reliable transportation
  • Able to work independently and as part of a team

Compensation and Benefits:

  • $165,000 annual salary
  • Monthly productivity bonuses

  • Medical licensure expenses reimbursed by employer
  • All equipment and practice expenses covered

Shift Structure:

  • Daily shifts from 7AM-1PM local time
