About the Role

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at the Community Based Outpatient Clinic (CBOC) in Kingston, NY. In this critical role, you will be responsible for delivering exceptional patient care, working collaboratively with our multidisciplinary team, and contributing to the achievement of our organization's mission.

Key Responsibilities:
  • Patient Care: Provide prescribed medical treatment and personal care services to patients with diseases and injuries, as directed by a physician or mid-level provider.
  • Collaboration and Communication: Work effectively with the core PACT Team, expanded PACT Team, and other healthcare professionals to develop patient care plans and ensure seamless coordination of care.
  • Clinical Skills: Perform routine nursing assessments, administer medications, dress wounds, and collect lab specimens, among other responsibilities.
Requirements:
  • Education: Diploma or certificate from an accredited program of practical or vocational nursing.
  • Experience: Minimum one year of experience as an LPN/LVN in a related primary care or ambulatory care setting.
  • Licensure: Current certification in Basic Life Support (BLS) and ACLS, as specified by individual VAMC contract.
  • Language: Proficiency in written and spoken English.
