The Clinical RN Navigator role at Central Health is a vital position that requires a highly skilled and compassionate nurse to navigate patients through complex healthcare environments. As a hybrid role, the successful candidate will be based in Austin, Texas, working from a call center and collaborating with various healthcare teams to optimize clinical outcomes.

Responsibilities
  • Collaborate with the patient's care team, including primary care physicians, specialists, virtual care providers, home care teams, and hospital staff, to ensure seamless transitions of care and optimal clinical outcomes.
  • Review clinical progress and coordinate communications with referring, consulting, and primary care physicians to ensure timely and effective decision-making.
  • Schedule appointments as needed and assist with the referral process, tracking outcomes of referrals and treatments.
  • Educate patients and family members on diagnoses, treatments, procedures, and medications in a call center setting, utilizing excellent customer service skills and empathy.
  • Support transitions of care activities with the patient's medical home care management team or the Central Health case management team, addressing social determinants of health and behavioral health needs.
  • Enhance the patient experience by supporting provider teams with in-basket management, refilling medications, and triaging patients in a timely manner following established protocols.
  • Participate in continuous quality improvement projects to better serve patients, families, and the healthcare system, improving the quality of services provided.

**Key Performance Indicators (KPIs):** Avoidable ER visits, hospital admissions, and readmissions will be closely monitored and addressed through targeted interventions.

Requirements
  • A minimum of 2 years of nursing experience in an ambulatory clinic and/or hospital setting.
  • An Associate's Degree in Nursing or higher (BSN preferred).
  • Certifications: RN - Registered Nurse, State Licensure and/or Compact State Licensure, Basic Life Support (BLS) obtained through approved American Heart Association or American Red Cross.
