Clinical Care Coordinator RN for Patient Navigation

We are seeking a highly skilled Clinical Care Coordinator RN to join our team at Central Health. As a key member of our transitions of care program, you will play a crucial role in ensuring seamless patient navigation and coordination across the healthcare continuum.
About the Role:This is a challenging yet rewarding opportunity for a seasoned RN with experience in primary care settings or ambulatory clinics. You will work closely with patients, families, and care teams to develop personalized discharge plans, educate patients on their conditions and medication regimens, and coordinate post-discharge follow-up appointments.
Key Responsibilities:- Collaborate with patients, families, and care teams to identify high-risk patients and develop targeted interventions
- Conduct thorough assessments to determine patient needs and create effective discharge plans
- Educate patients on their conditions, medications, and follow-up care requirements
- Coordinate post-discharge appointments and communicate with outpatient providers
- Work closely with case management, care coordination, and other hospital teams to ensure smooth transitions of care
- Graduation from an accredited school of nursing
- Two years of experience in a primary care setting or ambulatory clinic
- Unrestricted license to practice as a Registered Nurse in the State of Texas
- Basic Life Support (BLS) certification
