About the Role

We are seeking a compassionate and skilled Home Health LPN/LVN to join our team at AccentCare. As a clinical specialist, you will play a vital role in providing high-quality patient care and support.

Responsibilities:
  • Provide professional nursing care to patients in their homes, ensuring a safe and comfortable environment.
  • Conduct thorough assessments, develop personalized care plans, and implement evidence-based interventions.
  • Collaborate with interdisciplinary teams to coordinate patient care and services.
  • Maintain accurate records and documentation, adhering to regulatory requirements and company standards.
Requirements:
  • CURRENT STATE LICENSE IN EACH STATE WHERE RECEIVING VERBAL MEDICAL ORDERS FOR LICENSED VOCATIONAL NURSE, OR REGISTERED NURSE STATE LICENSE OR COMPACT LICENSE IF APPLICABLE
  • Minimum of one year experience as an LVN or RN in a medical setting, preferably home health
  • Knowledge of state and federal and accreditation requirements
  • Data entry skills, excellent verbal, and written communication skills (especially telephone)
