Hospice Admission RN Role Overview

HopeHealth, a leading not-for-profit healthcare provider in Massachusetts and Rhode Island, seeks a compassionate Registered Nurse (RN) to join our Hospice Care Team. As a Hospice Admission RN, you will play a vital role in providing skilled assessments to determine patient eligibility for hospice services and complete admissions for eligible patients.

This is a full-time position with a competitive salary range of $75,000 - $90,000 per year, depending on experience. The schedule includes working 1:00pm-9:30pm, rotating weekend commitment every 4th week. In this rewarding role, you will:
  • Represent HopeHealth in assigned settings, showcasing expertise in the hospice benefit.
  • Present hospice to eligible patients and caregivers, highlighting the benefits of the service in caring for individuals with terminal diagnoses.
  • Perform initial assessments of patients and families to determine the impact of terminal diagnoses on their physical, functional, psychosocial, and environmental needs.
With a strong desire to join an organization committed to delivering exceptional patient-centered care, you should have a minimum of 1-3 years of clinical experience, excellent nursing assessment, clinical, and communication skills.
