Job Title: Patient Navigator

At JWCH Institute, we are seeking a highly skilled and compassionate Patient Navigator to join our team. As a Patient Navigator, you will play a vital role in providing exceptional support to our clients, ensuring a seamless and positive experience.

Key Responsibilities:

  1. Welcoming clients and visitors with a friendly and professional demeanor, setting the tone for a positive experience.
  2. Engaging in meaningful conversations with clients to identify their informational needs and providing assistance to clients of diverse cultural backgrounds.
  3. Establishing and maintaining effective communication channels with clients and the healthcare team to ensure a smooth and coordinated experience.
  4. Serving as a liaison between clients and the medical staff, facilitating seamless interaction and information exchange.
  5. Coordinating the dissemination of updates and changes from front/back office staff to ensure accurate and timely information flow.
  6. Assisting clients in completing necessary forms, streamlining administrative processes for enhanced efficiency.
  7. Offering comprehensive information on on-site services and promoting health awareness to clients.
  8. Educating clients about available benefits, including health coverage programs, and referring them to Enrollment Specialists for eligibility screening.
  9. Representing the clinic in matters related to the delivery of healthcare services, working closely with the Clinic Administrator.
  10. Collaborating with the outreach team to greet and receive new clients, actively participating in the intake process.
  11. Engaging in community outreach initiatives to broaden the clinic's impact and accessibility.
  12. Maintaining incentive program logs, ensuring accurate and up-to-date records.
  13. Facilitating the transportation of patients from the local hospital Emergency Department to the E. Hollywood clinic using public transportation.
  14. Attending general staff meetings to stay informed and actively participating in the clinic's collective efforts.

Requirements:

  1. Demonstrated experience in effectively engaging with clients and collaborating with staff from diverse socio-economic, ethnic, and cultural backgrounds.
  2. Exceptional communication skills, both verbal and written, to facilitate clear and concise interactions.
  3. Bilingual proficiency in English and Spanish is highly desirable.
  4. Flexibility and capability to travel between different sites as required by the role.
  5. Valid CA Driver License and clean driving record.
  6. Capacity to collaborate effectively with healthcare teams and contribute to a cooperative and patient-focused care environment.
  7. At least 1 to 2 years of experience in client support or community service roles.
  8. Willingness to work some evenings and weekends, reflecting a commitment to meeting the varied needs of our community and clients.

JWCH Institute offers competitive salaries for all positions. Employees working 30 hours per week or more are provided a monthly allowance which can be used towards medical, dental, and vision premiums.
