Job Title: Care Coordinator

Job Summary:

Care Coordinators play a vital role in ensuring seamless patient care at MLK Community Healthcare. As a Care Coordinator, you will serve as a liaison between patients, providers, and the healthcare system, ensuring that patient needs, discharge planning, and care coordination efforts are aligned with care management criteria.

Key Responsibilities:

  • Assist patients through the healthcare system by operating as a patient advocate and health systems navigator.
  • Coordinate continuity of patient care with external healthcare organizations and facilities.
  • Report care barriers and challenges to physician or designee.
  • Conduct comprehensive, preventive screenings for patients and/or assist all support staff in daily patient interactions as needed.
  • Participate in the implementation of the Enhanced Care Management (ECM) with the goal of ensuring that eligible Medi-Cal beneficiaries receive enhanced care management and coordination.

Requirements:

  • High School Diploma required.
  • Bachelor's degree preferred.
  • 1 year of customer services experience in a healthcare setting or similar.
  • Healthcare and/or Hospital experience preferred.
