Position Overview:
The Resource Center for Independent Living is seeking dedicated individuals with a background in working with Veterans or who are Veterans themselves to fill the role of a Person-Centered Counselor.

Key Responsibilities:
The ideal candidate will be responsible for the following:
1. Collaborate with Veterans to create a detailed, individualized spending plan.
2. Acknowledge receipt of referral documentation promptly, within one business day.
3. Initiate contact with the Veteran within two business days to arrange an initial home visit.
4. Conduct quarterly in-person check-ins to assess the Veteran's well-being and review the spending plan.
5. Maintain effective communication with the Veteran and/or their Authorized Representative, providing guidance when there are discrepancies in adhering to the spending plan.
6. Continuously evaluate, identify, coordinate, and oversee necessary supports and services.

Qualifications:
Applicants should possess a valid NYS driver’s license, reliable transportation, and a willingness to travel as needed. A flexible schedule is essential to meet the needs of Veterans.

Educational Background:
A degree in Social Work is preferred; however, other relevant qualifications in Human Services will be considered. Relevant experience may substitute for formal education.

Essential Skills and Competencies:
- Proficiency in Microsoft Office Suite for document preparation, data management, and spreadsheets.
- Strong sensitivity and awareness regarding Veteran-related issues.
- Ability to engage with a diverse population.
- Skills in community networking, collaboration, and advocacy.
- Willingness to maintain a flexible work schedule.
- Excellent customer service, communication, and time management skills.
- Knowledge of human services, Independent Living philosophy, and case management is advantageous.
